This notice appears on every scan page, at signup, and on packaging. At signup every member affirms: “I understand HealthQR is an information service, not a medical provider, and I am responsible for the accuracy of my information.” We store the time of that consent and the version of the terms it was given against.
HealthQR does not verify the identity of people who scan a code, the accuracy of the information a member publishes, or the credentials of responders beyond the reasonable checks described in the Terms. If you are a responder, treat profile information as a member-supplied aid, not a clinical record.
